Quick checklist

  • Confirm legal name and DBA consistency across records.
  • Save operating authority and USDOT registration details.
  • Check whether the MCS-150 biennial update is current.
  • Keep insurance policy and filing documentation available.
  • Save UCR registration proof if applicable.
  • Review lease and ownership documents for equipment in service.

Why this matters

Audit packets get messy when authority, insurance, and registration records use different names or old addresses. The file should show who the carrier is and what authority and coverage apply.

What to prepare

Area Records to gather
Carrier records
  • USDOT number information
  • Operating authority details where applicable
  • MCS-150 review notes and biennial update status
  • Legal name and DBA records
Insurance and registration
  • Insurance policy or certificates
  • FMCSA insurance filing references where applicable
  • UCR proof where applicable
  • Equipment leases or ownership records

Common gaps

  • Insurance certificate shows a previous address.
  • Authority documents are saved under an old company name.
  • MCS-150 update status is not checked because company details did not change.
  • UCR receipt is missing.
  • Equipment lease records are kept by accounting only.

Before / During / After audit

Before

  • Compare names and addresses across records.
  • Ask the insurance agent for current certificates.
  • Save registration receipts in the audit packet.

During

  • Provide authority and insurance records together.
  • Use current records and note old names only when needed.

After

  • Set renewal reminders.
  • Update records after legal name or address changes.
  • Keep a current carrier profile PDF.
